Additional Information

Depending on your selections, we may require the following additional information. Please have the following information ready should our technicians require it complete the transfer of your site:

Any additional domain names
IP Address (If you have a static IP)
Login/Password For FTP Access if seperate from host login
Control Panel URL (Login/Password if different from FTP Login/Password)
IP Address (If you have a static IP)
Login Information for your domain names at your domain name registrar. (if you wish for us to make the DNS modifications for you)
If you have a mySQL or msSQL database setup that requires moving we'll need the SQL server IP or hostname, the name of the database(s) and the login information for the database(s).
Any URL's you've been using to access MIVA, or any other shopping cart you may be using.

If you do not have a control panel on your existing hosting account we will need a list of email accounts if any you need moved.
